$220 for 60-90 minute initial consultation

$120  for 30-60 minute follow-up visits

  • Every 3-6 months
  • Reevaluation of symptoms
  • Revision of care plan, medication adjustment or refills

$380 for exam, labs, follow-up for completion of I-693 Form
