Immigration Exams
Dr. Amy Schroeder, a USCIS-authorized Civil Surgeon, can perform your immigration medical exam and complete the required I-693 form. She does not work for the government, but Dr. Amy is committed to helping you on your path to becoming a United States resident or citizen. She is a board-certified family physician, speaks both English and Spanish and provides care for all ages.
Exam Fee: $380
Note: The fee does not include
the cost of follow-up tests or vaccines
What’s Included:
- Initial exam and review of medical documents
- Required blood and urine tests done on site
- Coordination of any needed vaccinations or follow-up testing
- Final visit to complete and sign the I-693 form
- Waived enrollment fee for Direct Primary Care membership
- Up to 3 followup visits to manage new medical issues, $80 per visit
Vaccine Requirements
Several vaccines are required for the I-693 form. Please bring proof of childhood vaccinations, and be prepared to:
- Provide a blood test showing immunity
- Receive any needed vaccines
If you have no records, the required vaccines will cost $150 minimum.
We provide these vaccines onsite for adults:
- Hepatitis B $80
- Tdap $60
- Polio $60
- Flu $30
These vaccines need to be obtained through a pharmacy:
- MMR
- Varicella
- Childhood doses of any vaccine
If you have health insurance, it may cover these vaccination costs.
What to Bring to Your Appointment
- Vaccine records
- Tuberculosis test or treatment records (if applicable)
- Any other relevant medical records
- Government-issued ID (driver’s license or passport)
- A translator, if you speak a language other than English or Spanish
